Coronary Heart Disease (CHD) care falls into two areas, newly diagnosed and ongoing help and advice for patients with heart problems. Our practice nurses provide the majority of this care, supported by the doctors. Various tests are performed to make the initial diagnosis, for example electrocardiograms (ECGs) and blood tests and once confirmed, advice regarding medication and lifestyle are offered. Ongoing monitoring involves a recall system and a personalised treatment plan for each patient.
